 本研究的目的為瞭解住在安、養護機構的老人之社會排除情形,以及影響其社會排除差異之因素。對象為居住在台北縣市安養護機構,年滿65歲的老人。抽樣分 為兩階段,第一個階段是針對台北市內安養護機構進行隨機抽樣;第二階段是針對台北市機構住民人數超過19人以上的機構進行立意抽樣,由於樣本不足,增加台 北縣兩所大型的安養及綜合型機構的抽樣。最後,取得有效問卷共327份。

本研究針對(一)文化與社交活動參與、(二)公共事務參與、(三)物質資源使用、(四) 鄰里社區融入、(五)服務使用、(六)財務生活融入、以及(七)社會關係等七個面向分別進行最大變異數轉軸的因素分析,以建構老人的社會排除(融入)更細 膩的指標。研究發現老人的部份社會人口屬性與社會排除確實相關,例如性別、教育程度,收入以及子女數等。老人的身心健康對於其社會排除也有顯著的影響,例 如憂鬱程度、自覺健康以及感官活動力。此外,機構本身的特質和服務也會影響老人的社會排除。最後,針對機構老人社會排除的現象提出政策與實務的建議,以增 進老人在機構和社區的融入。

The objectives of this study intended to investigate the phenomena of the social exclusion among the elderly residents in long term care institutions and to examine the factors which result in different exclusions. Survey research with face to face interview was employed. Perceptions and experiences of 327 institution-dwelling elderly persons using structured questionnaires were collected. The subjects are collected from long-term care institutions in Taipei City and Taipei County. To screen out elderly residents who were incapable of engaging in daily activities due to mental or physical disabilities and to enhance the validity of this study, only those subjects who were lightly disabled (ADLs scores more than 75) and cognitively clear (SPMSQ scores more than 8) were included. The researcher constructed items of ‘The elders’ Social Exclusion Scale’ according to the seven indicators or concepts of the British ELSA study of social exclusion: (1) Cultural and Social Inclusion, (2) Public Affairs Participation, (3) Material Goods Utilization, (4) Neighborhood and Community Inclusion, (5) Services Utilization, (6) Financial Management Inclusion and (7) Social Relationship. The original scales contained 80 items. Item analysis and factor analysis were employed to determine the structure or dimension of each domain. Finally, 70 items with an alpha measure of consistency of . 928 were included in the present study. The weighted averages of each dimension were applied in following analyses. The average age of the elderly is 82.12 for the study, and the average residing time is 4 years. Male residents are slightly more than female residents. Approximately 50% of respondents are widowed and over 60% are Mainlanders. The elderly are mostly excluded from Public Affair Participation and Cultural activities Participation. The findings indicated that there are significant correlations between some of the elders’ social-demographic characteristics and social exclusion, such as gender, education, income and number of children. The physical and mental health status, for instance, depression, sensory and mobility, and self-evaluate health also have significant influence. Additionally, some characteristics of the institutions and service provided by the institutions have their effects as well. Finally, policy and practice implication toward the elders’ social exclusion were discussed to improve their inclusion in the care institutions and communities.
